Investigating accidents, incidents and near-misses

All adverse events, whether or not they result in injury or damage to property or equipment, should be investigated by the local manager to determine the cause and decide what action should be taken to prevent it happening again. The local safety advisor may help with the investigation.

Please note: The Main Investigation Form is to be used by default. However, it has been agreed that the Initial Investigation Form can be used by certain areas. Please discuss with your Faculty/Directorate health and safety lead contact which form is appropriate for your area.
